I think you have to look at the different levels of quality

I agree. How do you see the two cultures on each of the levels?

Steve, something I believe very relevant occurred to me as I considered my
response:

Modern man's civilized life is possible due to some people doing the "dirty"
work.

Would you agree that there is a very negative level of biological quality to
being in a dark mine shaft all day with the high probability of lung related
disease and accidental death?  I know the figures for coal mining deaths
were horrendous when it first began and I believe I heard recently 6,000 a
year still die in China in mining accidents.  We could cite countless other
examples of this "dirty" work throughout history, a lot of it involving
slavery.

I think it is reasonable to speculate that some of these people doing these
jobs might prefer primitive man's life in the natural setting with fresh air
and the freedom to move about.  They might well find hunter/gathering a more
emotionally satisfying way to gain sustenance then the dreadful conditions
they work under now.  A more satisfied emotional state I believe would come
under biological quality.

So these people might be willing to trade off the anesthesia for the lung
cancer operation they wouldn't need in the first place if they weren't doing
there work for modern life's sake.

And that's even if they can get an operation at the free clinic.
