This quote only mentions two elements, but that doesn't mean there are "just two elements".
IMHO concepts and reality are not the primary distinction in the MoQ.  That would be DQ/sq.
Within sq there are concept spov and non-concept spov.

A banana is a biological spov, not a word (see use/mention distinction: mice have 4 legs; 'mice' has 4 letters.)
There is a word 'banana' that is a 4th level spov, but it is not edible.
